I always thought it was condensation on the inside of the packaging...but, I could be wrong!

It's caused by dehydration of the food, caused by exposure to air. It is perfectly fine to eat, it will have different texture and possible flavor change.
Be sure to wrap your items carefully, removing as much air as possible from plastic bags, foil wrapped items should be wrapped as tight as possible. Be sure to leave room at the top and sides of the freezer for air to circulate with in the freezer.

Yep - if there's frost somewhere inside your package, that water came from somewhere.

If you suck all of the air out of the container first, you won't have freezer burn. If you're packing things in ziplocks, use a straw and suck out as much air as possible. Wrap solid foods like meat, breads, etc. tightly in plastic wrap and then in foil or butcher paper.
